Adolescentie is in de levenscyclus van de mens een vrij unieke periode. Het is een fase die gekenmerkt wordt door fysische, psychosociale en cognitieve ontwikkeling. De overgang in de ontwikkeling tussen kind en volwassene, is een groeifase met een verhoogde behoefte aan nutriënten. Deze verhoogde eisen zijn waar te nemen in een gewichtstoename (tot 50% van het volwassen gewicht), in groeischeuten (tot 20% van de volwassen lengte) en in een toenemende sterkte van het beendergestel van adolescenten. Adolescentie wordt gekenmerkt als de periode in de levenscyclus waar het vroegere en huidige voedingsgedrag kan aangepast worden. Het bijsturen van voedings- en leefgewoonten is dan ook een belangrijk aangrijpingspunt in de preventie van voedingsgerelateerde chronische aandoeningen. In kader van het ‘First Action Plan for Food and Nutrition Policy of the World Health Organization – European Region’ werd ook aandacht besteed aan het luik voedselveiligheid bij adolescenten. In dit proefschrift wordt de betekenis van voeding – op nutritioneel en voedselveiligheid vlak – voor de gezondheid van adolescenten onderzocht. Voor de analyses is gebruik gemaakt van de gegevens van de ‘Ghent Adolescent Study On Nutrition’. Deze dataset bevat gegevens over voedingsgewoonten van Vlaamse adolescenten, tussen 13 en 18 jaar. Het vernoemde project maakte deel uit van een internationaal project met een focus voor methodologische aspecten van voedselveiligheid onderzoek.

Adolescence is a particularly unique period in life as it is characterized by intense physical, psychosocial and cognitive development. From nutritional point of view, this transition period from childhood to adulthood deserves special attention in view of the remarkable physical changes of the body. Increased nutritional needs relate predominantly to the fact that adolescents gain up to 50% of their adult weight, more than 20% of their adult height and 50% of their adult skeletal mass during this period. During adolescence, nutritional problems originating earlier in life can potentially be corrected, in addition to addressing current ones. It is also a period to shape and consolidate healthy eating and lifestyle behaviours, thereby preventing or postponing the onset of nutrition-related chronic diseases in adulthood. In the context of the “First Action Plan for Food and Nutrition Policy of the World Health Organisation - European Region” attention has been paid on the issue of food safety. This thesis aimed to identify nutritional and food safety issues that contribute to adolescents’ health. For the analyses, data of the ‘Ghent Adolescent Study On Nutrition’ were used. This study includes data on dietary pattern of Flemish adolescents, between 13 and 18 years old. The study was part of an international collaborative project, looking at methodological aspects of dietary assessment in the context of food safety monitoring.

Gezondheid en welzijn:Jeugdhulp:Hulp aan (kinderen in) gezinnenSamenvatting: JOnG! is een algemeen longitudinaal onderzoek naar ontwikkeling, opvoeding, gezondheid en gedrag bij 0-, 6-, en 12-jarigen en hun gezin. Ook de hulp die aan de gezinnen verstrekt wordt, wordt in kaart gebracht. Er wordt gebruik gemaakt van een gemengd longitudinaal design, waarbij drie cohorten gevolgd worden over een periode van 3 jaar (een cohorte van 0-jarigen, 6-jarigen en 12-jarigen). De steekproeftrekking gebeurt in twee fasen, waarbij uitgegaan wordt van een ‘conditional random sampling plan’. In de eerste fase worden over de verschillende programma’s en onderzoekslijnen van het Steunpunt Welzijn, Volksgezondheid en Gezin heen negen regio’s geselecteerd. Deze negen geselecteerde regio’s volgen de regio-omschrijving die Kind & Gezin voor haar consultatiebureaus toepast. Elke regio vertegenwoordigt een geboortecohorte van ongeveer 1000 kinderen. Bij de selectie wordt rekening gehouden met de principes diversiteit (sociaal economische kenmerken), stedelijkheid en provinciale spreiding. Met betrekking tot de eerste twee principes werden zowel gemiddelde als eerder uitzonderlijke regio’s opgenomen. In een tweede fase worden, specifiek voor JOnG!, drie cohorten geselecteerd voor verder onderzoek. Voor de 6- en 12-jarigen wordt de volledige cohorte geselecteerd. Voor de 0-jarigen wordt ongeveer de helft van de cohorte geselecteerd. Summary: JOnG! (in English: ‘Young!’) is a general longitudinal research project regarding the development, education, health and behavior among children aged 0, 6 and 12 years as well as their families. Furthermore, the care these families receive is topic of the study as well. Respondents from three cohorts (children aged 0, 6 and 12 years) are followed over a period of 3 years. To select the sample, a two-phased methodology is used, following a ‘conditional random sampling plan’. In the first phase, nine regions are selected from the different programmes and research groups of the Policy Support Centre of Well-being, Health and Family . Every region represents a cohort of more or less 1000 children. In the selection process, attention is being paid to diversity (regarding socio-economic qualities), urbanism and equal distribution between provinces; regarding diversity and urbanism, average as well as extreme regions are being selected. In the second phase of the selection, three cohorts are selected for the JONG! Sample. In the groups of youth aged 6 and 12 years, the complete cohort is being selected. As for the children aged 0 years, only half of the cohort is selected. Gezondheid en welzijn:AlgemeenSamenvatting: Zoals geformuleerd in het Kinderrechtenverdrag, aangenomen door de Verenigde Naties, hebben kinderen recht op zowel een gezonde leefomgeving, leren gezond te leven als recht op een kwalitatieve gezondheidszorg. Uit de bepalingen van het verdrag kan ook worden afgeleid dat een focus moet worden gelegd op preventie en gezondheidsbevordering en dat hierbij de sociale en omgevingsdeterminanten van gezondheid niet uit het oog mogen worden verloren. Deze uitgangspunten voor gezondheidsbevordering vormen een belangrijke uitdaging voor het leveren van evidence voor een gezondheidbevorderende praktijk en beleid. In deze bijdrage wordt geargumenteerd dat een strikte interpretatie van evidence-based beleid en praktijk niet haalbaar is in het brede domein van de gezondheidsbevordering. Het alternatief, het realiseren van een evidence-informed beleid en praktijk, is geen lineair proces dat uitsluitend gebaseerd is op resultaten van experimenten, maar houdt daarnaast rekening met het perspectief van alle belanghebbenden in de gezondheidsbevordering. De beschikbare evidence moet gemakkelijk toegankelijk zijn voor iedereen en het beslissingsproces moet transparant verlopen. Door het hanteren van het raamwerk van de kinderrechten voor het maken van keuzes wordt het belang van het gehele kind en zijn of haar levensomstandigheden mee in overweging genomen. Summary: The Convention of the Rights of the Child states that children are entitled to as well a healthy environment, learning to live healthily as a right to quality health care while taking into account the social and environmental determinants of health. These principles are a major challenge to provide ‘evidence’ for a health promoting practice and policy. It is argued that a strict interpretation of evidence-based policy and practice is not feasible in the broad field of health promotion. The alternative, an ‘evidence-informed’ policy and practice, is not a linear process based solely on the results of experiments, but a transparent process that also takes into account the perspective of all stakeholders in health promotion interventions. By applying the framework of the rights of children to make choices for policy and practice, the importance of the whole child and his or her living conditions will be taken into consideration. Om de vier jaar wordt in een toenemend aantal landen (laatste ‘survey’ dateert van 2009-2010: 41 (vooral Europese) landen of regio’s) dezelfde enquête afgenomen volgens een internationaal protocol. De eerste Vlaamse enquête dateert van het schooljaar 1989-1990. Het doel van de studie is inzicht verkrijgen in de gezondheid, het gezondheidsgedrag en de sociale context van jongeren tussen 11 en 18 jaar (bevraging van 5de leerjaar lager onderwijs tot en met 6de jaar secundair onderwijs). Aangezien deze studie het monitoren van het gezondheidsgedrag van jongeren als doel heeft, wordt het internationaal protocol telkens aangevuld met vragen die van lokaal (Vlaams) belang zijn. Het Vlaamse HBSC-team kan beschikken over de internationale databank zodat vergelijkingen met andere landen mogelijk zijn. De thema’s die bestudeerd worden in de studie zijn levensstijl (zoals voeding, roken, alcoholgebruik, druggebruik, seksualiteit, fysieke activiteit), subjectieve gezondheid en psychisch welbevinden, en de omgeving van de jongere (gezin, school en buurt). De studie bezit vijf concrete doelstellingen: (1) inzicht verwerven in gezondheidsgerelateerd gedrag van jongeren en determinanten ervan; (2) monitoring van de evolutie van het gezondheidsgerelateerd gedrag en hun determinanten; (3) informatie verstrekken aan het veld; (4) formuleren van strategieën en concrete voorstellen die het gezondheidsgedrag in de doelgroep kunnen wijzigen; en (5) onderhouden van een informatiesysteem over gezondheidsgerelateerd gedrag van jongeren. Summary: The study ‘Youth and health’ is part of the international study ‘Health Behaviour in School-aged Children’ (HBSC)(www.hbsc.org) which is carried out by the World Health Organisation. Once every four years the same survey is executed within an increasing amount of countries (the last survey dates back from 2009-2010 with 41 (mostly European) countries or regions) according to an international protocol. The first Flemish (Belgian) survey dates back from the year 1989-1990. The purpose of this study is to gain insight into the health, health behaviour and social context of youth between the ages of 11 and 18 years (survey is taken from children in the 5th grade up until children in the 12th grade). Due to the purpose of monitoring the health behaviour of youngsters, the international protocol is constantly replenished with questions of local (Flemish) importance. The Flemish (Belgian) HBSC team is able to access the international database so that comparisons with other countries are possible. The studied themes concern lifestyle (such as food, smoking, alcohol use, drug use, sexuality, and physical activity), subjective health and psychical well-being, and the environment of youngsters (family, school, and neighbourhood). The study consists of five concrete objectives: (1) to gain knowledge into the health behaviour of youth and its determinants; (2) to monitor the evolution of health behaviour and its determinants; (3) to provide information to the field; (4) to formulate strategies and concrete recommendations to alter the health behaviour of the target group; and (5) to maintain an information system about the health related behaviour of youngsters.
